Please prepare all your requirements and submit the same at the Registrar’s Office where the course is offered. (Check the Campus and Curricular Offerings)

Arrange the documents/credentials in the same sequence stated below for easy checking during enrollment period.

REQUIREMENTS:

Incoming First Year Students:
1. Entrance Examination Result c/o Guidance Office (SYSTEM)
2. Report Card/ Form 138/ SHS Report Card (Original)
3. Certificate of Good Moral Character (Original)
4. NSO/PSA Authenticated Certificate of Live Birth (Photocopy)
5. 2pcs. 2×2 picture (recent, identical, and with nametag)

Transferee:
Same requirements as with first year students, but instead of Report Card/SHS Report Card (no. 3), submit the following:
1. Transcript of Records with remarks: Student’s Copy or Evaluation (Original)
2. Certificate of Transfer Credential / Honorable Dismissal (Original)

Enrollment Period will start on July 4, 2024
